How to Use Virginia’s Inspection Database When Choosing Assisted Living
Why inspections matter. Online reviews and star ratings capture personal experiences, but Virginia’s Department of Social Services (VDSS) inspection records show what regulators found on site. Reading them alongside tours helps families make confident, apples-to-apples decisions.
Step 1: Find the facility. Open the VDSS Assisted Living Facility search and type the community name or city. If multiple results appear, choose the full legal name and address that match your tour list.
Step 2: Check the date and type. On the profile page, look at the most recent inspection. Note whether it’s routine (regular visit), complaint (triggered by a concern), or follow-up (verifying corrections).
Step 3: Scan findings. Read the summary of findings. Is there a pattern (e.g., repeated medication issues, staffing documentation gaps, or building safety items)? One-off issues that were quickly corrected can be less concerning than repeated, unresolved findings.
Step 4: Look for corrections. Many reports document how and when the provider corrected an issue. Verified corrections and clean follow-ups indicate the community’s ability to improve.
Step 5: Compare across your shortlist. Put inspection notes next to your personal priorities. For example, if memory care programming is critical, look closely at staff training and care-planning findings. If mobility support matters, review fall reporting and call-bell response documentation.
Step 6: Bring questions to the tour. Use inspections as a conversation starter, not a verdict. Ask: “Can you share how you addressed the medication findings from March?” or “What changes have you made since the last staffing audit?”
What’s a red flag? Repeated, unresolved violations across multiple inspections; recent complaint-based findings involving resident safety; or a lack of documented corrections. Use your judgment and weigh the overall pattern.
